Description of role

• Care for 8- 20 (maximum), patients continuously- manage long term care plans
• Majority of patients are children, young adults, geriatrics and dependants on NDIS
• Care takes place in the patient's home with their carer present
• You will be able to work with complete autonomy and enjoy the support of 2 lead GPs
• All patients are located central to Chatswood- 30 min commute
• You will oversee 5- 6 Care Managers and ensure they receive the guidance/ supervision required
• Train and support positive patient outcomes in hand with a GP and your care manager team
• Visit 2- 3 homes per day (1- 3 residents in each home) 

• Headquarters located in St Leonards-  clinical staff have a weekly/ fortnightly meeting
• Varying complexity of care- continue developing your clinical capabilities

 
Experience sought

• Practising Registered Nurse of 4 years +
• Experience/ exposure to community/ aged care
• Interest/ experience in team leadership
