Description of role

· Care for 20- 25 (maximum), patients continuously- develop long term care plans
· Majority of patients are children, young adults and dependants on NDIS
· Consults take place in the patient's home with their carer/ nurse present
· You will be able to work with complete autonomy and enjoy the support of 2 lead GPs
· All patients are segmented into primary areas of Sydney- North, South, West.
· All software and technology is utilised to make your day smoother
· Clinic head quarters located in St Leonards- GPs use this as their office if desired
· Best Practice software
